We make sure all our new patients feel welcome, safe, and taken care of. During your first visit, we will review your medical history.
For a shorter time in the waiting room, please fill out these new patient forms and bring them to your visit. You will also need to present a valid driver's license and your medical provider information card at the time of check-in.

We are not accepting new Medicare patients at this time. As our current patients become eligible for Medicare, our practice will gladly continue care and file to Medicare.
We do not take or accept any Market Place insurance plans.
If we are not on your plan, we request that you pay at the time services are rendered and file to your own insurance company. We will provide you with the information required to file a claim with your insurance provider.
We accept Cash, Personal Checks, Visa, Master Card, Discover, and American Express.
Charges for office visits and procedures are determined by time spent and the severity/complexity of the problem. Payment is due at the time of service.
